Sorry for hijacking this thread, but I have to elaborate on this because of your special setup (XS 6.5 and using three iSCSI-SRs).
Aside from that you may be hit by a quirk in the 'replug_pbd' script, which leads to reattachement of only two iSCSI-SRs after rebooting the pool/a host.

@Tim @sc/@Pulse Supply: Could you please try this in your environment and/or take a look at the code inside the 'replug_pbd' script if this holds true?
Possible fix enclosed.

Nice catch! You are correct. Looks like the script was hardcoded to deal with only 2 SRs

Your fix has been rolled into our current development version that addresses a few minor issues.

tested this in my production pool with 3 sr's and my lab pool 4 sr's works like a charm in both cases. thank you.
